American Cancer Society has a program available to those who qualify to assist you in purchasing a wig. If your
insurance does not cover the full cost of your wig or only covers partial cost of your wig, you may quality for an
allotment through the American Cancer Society. The American Cancer Society will issue you a voucher up to
$75 to help cover the cost of your wig. You must meet certain financial guidelines. You MUST apply for this
voucher BEFORE you purchase a wig. It will not be awarded for pre-made purchases.

If you file an insurance claim for reimbursement for a "wig" instead of a "cranial hair prosthesis" you will very
likely be turned down? Most insurance companies cover between 80 to 100% of a cranial prosthesis?
You can use the following guidelines to file your claim to the insurance company:
1) Check the benefits of your insurance. There should be a section under "covered expenses" relating to
prostheses, prosthetic devices or medical durable equipment. Most companies don't spell out exclusions so
pay attention to the actual coverage. This will help your case. You can contact your insurance company find
out if you have this coverage, ask your insurance company do you have coverage for " Cranial Hair Prosthesis."
This is the medical term under which lace front wigs are filed. When you contact your insurance carrier to see if
eligble, you may also ask them, are you eligible for a "Cranial Hair Prosthesis "under prescription code S8095;
this is the prescription code that would be assigned by your physician if you are eligible.
2)Request that your physician write a "prescription" for a "full cranial prosthesis" (not a wig) and a letter
explaining hair loss and its effects. That it isn't just for cosmetic reasons but for emotional well being.
3) Notify us about your need for alternative hair and we will send you a receipt for a Cranial Prosthesis.
4) Make sure to Send the sales receipt for a Cranial Prosthesis to your insurance company, along with your
completed insurance form and have it signed by your physician.
5) Keep copies of your sales or invoices for your cranial prosthesis (Portions of unpaid items by
insurance are tax deductible.) for insurance reimbursement, or upfront insurance payment.
