American Cancer Society as well as other organizations. To see if you qualify for a special grant to pay for your
Full Cranial Prosthesis (full lace wig) Below are instructions on how to apply for a grant or voucher for your
wig. Some organizations also refer to these as full cranial prothesis.

ASCOT FUND Grant
Thanks to an anonymous donor the National Alopecia Areata Foundation (NAAF) established the Ascot
Fund. This special pre-purchase program provides financial assistance toward the purchase of a hairpiece to
individuals who have alopecia areata and are struggling financially.
You are eligible to apply for assistance if you:
• have been diagnosed with alopecia areata by a dermatologist or other physician.
• are unable to purchase a hairpiece due to financial challenges.
• are not in possession of the hairpiece for which I am requesting assistance to purchase.
• have not already paid for the entire cost of the hairpiece with a credit card or other means.
• can pay for any balance that remains within 30 days of submitting your application should you receive a
maximum award.

American Cancer Society:
American Cancer Society has a program available to those who qualify to assist you in purchasing a wig. If
your insurance does not cover the full cost of your wig or only covers partial cost of your wig, you may quality for
an allotment through the American Cancer Society. The American Cancer Society will issue you a voucher up
to $75 to help cover the cost of your wig. You must meet certain financial guidelines. You MUST apply for this
voucher BEFORE you purchase a wig. It will not be awarded for pre-made purchases.

You may qualify to receive health insurance reimbursement, or upfront health insurance payment. For
persons who have insurance and are purchasing (Lace Front Wigs) for medical conditions, you may be
covered for what the insurance companies call “hair prosthesis”.
If a person requires a "cranial hair prosthesis" while undergoing Alopecia or
cancer-related therapy, they may be eligible for reimbursement under their health insurance plan?
If you need to wear a wig because of medical reasons like Alopecia and cancer-related therapy then the "wig"
should be called a "cranial hair prosthesis" particularly for insurance purposes?
If you file an insurance claim for reimbursement for a "wig" instead of a "cranial hair prosthesis" you will
very likely be turned down? Most insurance companies cover between 80 to 100% of a cranial prosthesis?
Insurance Companies are realizing your cranial hair prosthesis are more and more a necessity for
those recovering from Alopecia and cancer treatments. Follow some simple guidelines to assure
coverage, when it is available through your insurance company. Your physician should be able to
supply all the details and make the referral for the purchase of your "hair prosthesis". Ask your
physician to do that.
You can use the following guidelines to file your claim to the insurance company:
1) Check the benefits of your insurance. There should be a section under "covered expenses" relating to
prostheses, prosthetic devices or medical durable equipment. Most companies don't spell out exclusions so
pay attention to the actual coverage. This will help your case. You can contact your insurance company find out
if you have this coverage, ask your insurance company do you have coverage for " Cranial Hair Prosthesis."
This is the medical term under which lace front wigs are filed. When you contact your insurance carrier to see if
eligble, you may also ask them, are you eligible for a "Cranial Hair Prosthesis "under prescription code S8095;
this is the prescription code that would be assigned by your physician if you are eligible.
2)Request that your physician write a "prescription" for a "full cranial prosthesis" (not a wig) and a letter
explaining hair loss and its effects. That it isn't just for cosmetic reasons but for emotional well being.
3) Notify us about your need for alternative hair and we will send you a receipt for a Cranial Prosthesis.
4) Make sure to Send the sales receipt for a Cranial Prosthesis to your insurance company, along with your
completed insurance form and have it signed by your physician.
5) Keep copies of your sales or invoices for your cranial prosthesis (Portions of unpaid items by
insurance are tax deductible.) for insurance reimbursement, or upfront insurance payment.
Always call your hair system a Cranial Prosthesis! NEVER refer to it as a WIG.
Cranial Prosthesis are different from shelved store wigs in quality, performance and service. Prosthesis are
made from specialized material, for individual use for severe hair loss.
